Alberta Health Services is encouraging community members to review and participate in the decision making process concerning their ADVANCE CARE PLANNING and GOALS OF CARE DESIGNATIONS (documents also known as “The Green Sleeve”).
“Advance Care Planning is a way to help you think about, talk about and document your wishes for health care. It is a process that can assist you in making health care decisions now and for the future. Advance Care Planning may bring comfort and peace of mind to you, your family, and to those who may have to make healthcare decisions on your behalf.”
“Goals of Care Designations are instructions that guide your health care team about the general focus of your care, and where you might wish to receive care.”
“The health care team will work with you to help choose Goals of Care Designation that best reflects your values and beliefs, your health conditions and appropriate treatment options. By knowing and following your Goals of Care Designation, healthcare providers care for you in a way that is timely, medically appropriate, and meets your personal values and wishes.”
Having your Advance Care Planning in place and understanding Goals of Care Designations is important to us all. Let’s be proactive concerning our own healthcare decisions!
On Wednesday May 3, 2017 at 1:30 - 3:00 pm at the Bragg Creek Snowbirds’ Chalet, an A.H.S. representative has been invited to give us a presentation on the Advance Care Planning/Goals of Care Designation process. A similar session was held in June 2016, and it was requested that an additional session be provided.
